Smile Paperwork

I did this task to make it easier for myself when it came to the production stage as filling out this paperwork ensured that I knew where I was recording, when I was supposed to be recording and that I had permission to be recording where I was going to record (which for me, was on campus.)


  Firstly, I filled out the Risk Assessment form which generally describes what I planned on doing during the production stage such as filming locations, the team members and what the general assessment was on (Which for me was a one day shoot across a few hours) and then goes it on to outline the potential risks/hazards that could occur during the filming. I then chose a few issues that could occur during filming such as there being a cold environment and the potenial issue that the actor could feel fatigued given that recording was to be done in a day. I then said in detail what I planned on doing to make sure that these issues were not an issue any longer or at least they were much less likely which is good as it means if anything were to happen during filming, I had the paperwork to prove that I had acknowledged the risks and had put things into place to ensure that the risk was as small as possible.

I also filled out a personnel list and an equipment list to make it easier for me when it came to production as I knew what equipment I had to get and where I needed to get as well as what equipment I needed. The personnel list also made it easier for me as it meant that if I needed to get in touch with the cast if someone didnt turn up then I had an easy way of contacting them.

I also filled out a location release form as this was proof that I had permission from the property manager to record on campus. I also filled out a location scout form as this went over imporant details for the location such as soundscape, power capabilities, if parking was available as well as other detials such as that there were facilities available if the cast and crew wanted to get food or something to drink during the breaks I planned on giving during the day.

Finally, I had consent forms filled out which were important seeing as having these signed meant that anyone who had been recorded and gave me their consent couldnt turn around later down the line and say I couldnt use any footage with them in it.
